What we aim to solve
Alliance Medical Ministry provides access to comprehensive primary care to working, uninsured adults in Wake County. As of the end of 2017, approximately 90,000 Wake County residents are uninsured. The uninsured are more likely to forgo regular medical check-ups, less likely to fill needed prescriptions, more likely to develop chronic illnesses, and more likely to die early. Alliance aims to be a medical home for uninsured adults living in our community, reducing emergency room usage, and increasing access to care to a vulnerable population. Access to healthcare remains a major issue, identified as one of the top four areas of concern in the 2016 Wake County Community Health Needs Assessment. Without a medical home, uninsured individuals have no access to affordable primary care, often visiting local emergency rooms for routine care, incurring up to $1500 for a non-emergency visit and episodic care.
Our programs
What are the organization's current programs, how do they measure success, and who do the programs serve?
Mental Health Program
The Director of our mental health program holds a Doctorate of Psychology and Masters of Divinity. The Director oversees a team of volunteers with backgrounds varying: marriage and family therapy, psychotherapy, divinity, social work. Patients are seen for depression, anxiety, substance abuse, domestic violence and intimate partner violence. For psychiatric issues, patients are referred to Turning Point Behavior in Wake County.
Healthy Steps Wellness Program
Through an innovative and comprehensive wellness program, Alliance Medical Ministry seeks to help patients manage chronic illnesses and prevent the onset of new illnesses. A majority of our patients are chronically ill; 55% have multiple chronic illnesses, including diabetes, hypertension, high cholesterol, obesity, and respiratory issues. Our Wellness Program includes:
-Fresh produce from our on-site Community Garden
-Six-week cooking classes, where patients learn how to meal plan, grocery shop, and cook healthy, affordable meals
-Diabetes Education
-Diabetes Prevention Program
-Diabetes Group Visits
-Weekly yoga
-Weekly Zumba
-Walking Program
-Garden education
Primary Care
Board-certified providers in Internal Medicine or Family Medicine provide the medical foundation for patients at AMM.
Upon acceptance as a patient at AMM, each individual is assigned to a medical provider, and continues to see that provider while a patient at our practice.
Care provided:
Chronic disease management and preventative care
Lab Work: Thanks to UNC-REX Healthcare, most lab work is done onsite after the patient’s appointment at no additional cost for patient.
Medications: With necessary financial information provided by patients, medication assistance may be available.

What is the organization aiming to accomplish?
Alliance Medical Ministry seeks to provide: (1) Primary and acute care and chronic disease management to uninsured adults through a staff physician; (2) Select specialty care by a volunteer provider (cardiology, gynecology, ophthalmology, physical therapy, orthopedics, wound card, ENT, endocrinology; (3) lab work onsite; (4) access to affordable prescription medications; (5) off-site referrals for screenings, procedures, and specialty care through Wake County Medical Society Project Access; (6) mental health and counseling program; (7) off-site psychiatric referrals; (8) Community Garden providing access to about 2,000 pounds of fresh produce; (9) comprehensive wellness program, including exercise, nutrition, gardening, and diabetes education to a chronically ill population. Alliance Medical Ministry aims to help our patients manage their chronic illnesses through consistent, quality primary care and access to medication, and prevent the onset of new illnesses.
What are the organization's key strategies for making this happen?
Alliance runs much like a private practice, with a full medical staff to address patient needs. This staff-based model is deliberate, helping us achieve continuity of care and build strong relationships with our patients. Alliance Medical staff includes three full-time medical providers, two nurses, two medical assistants, a phlebotomist, referral coordinator, and Patient Assistance Program coordinator. Our clinic model is team-based, with providers grouped with nurses and medical support staff in a clinic pod. Patients are assigned to one provider, and routinely see that provider at each visit. For patients, seeing the same care team builds familiarity and trust. Patients feel like their voices are heard and are empowered to participate in their care. In turn, our medical staff gains a deeper understanding of a patient's current health, medical history, lifestyle, and outside issues impact their health, strengthening the quality of care provided.
What are the organization's capabilities for doing this?
Alliance Medical Ministry has a trained and highly qualified medical staff, including: medical director, physicians, Doctor of Psychology, nurse practitioner, RNs, MAs, phlebotomist. Alliance is located near Wake Med Hospital off of New Bern Avenue in southeast Raleigh. Our clinic is open Monday through Friday, 8:30 AM to 4:30 PM. For a sliding scale co-pay based on household income (about $20-25 per visit), patients receive primary care from a staff physician, specialty care from volunteers (ENT services, gynecology, physical therapy, orthopedics, cardiology), select screenings (cardiac, mammograms, dental), off-site medical referrals through Wake County Medical Society's Project Access, on-site lab work, access to affordable prescription medication, chronic disease management, counseling and mental health services, off-site psychiatric referrals, health education, and access to a comprehensive Wellness Program. Alliance approaches medicine in a holistic manner, addressing the mind, body and spirit of our patients under one roof. We believe that in order to improve an individual's physical health, we must address their mental and emotional health needs and address unhealthy lifestyle behaviors. Alliance achieves this holistic care through our mental health and Wellness Program. Our mental health program is led by an individual holding a Doctorate of Psychology and Masters in Divinity. He has built a team of volunteers from a range of backgrounds (social work, marriage and family therapy, licensed professional counseling, Duke Divinity interns) to help our patients address anxiety, depression, substance abuse, domestic abuse, and other issues. Our Wellness Program is an innovative program in which our physicians “prescribe" fresh produce, diabetes education, cooking classes, yoga, and other exercise opportunities to help our patients prevent the onset of chronic illnesses, manage existing illnesses, change eating habits, and incorporate exercise into their daily lives.
What have they accomplished so far and what's next?
When Alliance Medical Ministry was formed in 2003, the largest gap in community healthcare coverage was for low-income working adults, those individuals who made too much money to qualify for government support but not enough to purchase private health insurance. Fourteen years later, even with national healthcare reform, that healthcare gap continues to exist, with about 90,000 Wake County residents uninsured at the end of 2016. The day the clinic opened, ten patients were seen by a two-person medical staff. Now, almost 3,000 patients are seen, over 10,000 patient appointments are performed annually, and our staff has grown to include three providers, two nurses, medical assistant, a phlebotomist, a chaplain, a garden and wellness coordinator, and an administrative team. Alliance's focus surpasses treatment of illness or injury; our medical care is holistic and compassionate in nature, healing the mind, body and spirit of our patients, ensuring long-term improved health outcomes.
