Mission Statement
Project PLASE addresses homelessness by providing transitional and permanent housing and supportive services to homeless adults, particularly the most vulnerable and under-served (i.e., people with mental illness, HIV/AIDS, addictions, developmental disabilities, ex-offenders, etc.) and by treating the whole person and empowering each individual to function at the highest possible level.

Impact Statement
Successes in Meeting Objectives
in the Permanent Housing in FY10:
· 97% of the clients we served through our Permanent Housing Program remained in housing at the end of the year. 37 of those who left moved to other types of permanent housing such as Section 8, Senior Housing, family homes or Assisted Living.
· 9 of the 37 clients who left PLASE were able to obtain their own homes or non-subsidized rental homes.
· High level of success connecting clients with much-needed services.
· Permanent Housing clients were 82% compliant with substance-abuse treatment.
· 91% of clients with mental health histories were at least 70% compliant with mental health regimens.
· 87% of clients were at least 75% compliant with their medical treatment regimens.
Successes in Meeting
Transitional Housing Objectives in FY10:
· 94% of the clients accessed community resources and services (mental health, substance abuse, medical needs, vocational training, benefits, etc.).
· 83% of all clients with reported substance abuse histories accessed treatment either in-house or in the community.
· 51% of the individuals in our Transitional Housing Program moved to permanent housing (either with PLASE Permanent Housing Program or alternative permanent housing).
· 63% of residents reported having a history of incarceration, yet less than 1.5% returned to jail upon disposition.

Chief Executive Profile:
Mary C. Slicher, LCSW-C, is one of the original founders of PLASE. She has been active on a number of boards of fellow non-profit organizations, as well as peace and justice organizations. She is currently pursuing a second Master's Degree at the College of Notre Dame. She has received several awards: Sarah Circle, The Women's Institute, Life Time Organizing Award, Peace and Justice Commission and P. Francis Murphy Award.

This year, Project PLASE pushed further ahead towards ending
homelessness and its lingering injustices for the numerous individuals coming
through our doors.
Baltimore is not
the only American city with homeless men, women and children living on the
streets, but our city’s homeless population suffers from a mixture of
devastating setbacks and personal challenges that call out for immediate
engagement, total compassion and delivery of effective, long-term solutions. For
this we need a dedicated and professional staff. PLASE staff provides our
clients with the clarity to assess their own situation, and space to make
positive and lasting resolutions. PLASE staff guides our clients into programs
that help counter self-destructive patterns, reverse years of serious physical
and mental health problems aggravated by lack of medical care, and reconstruct
the desire and ability to engage with our neighbors and larger community. PLASE

Program Description:
We provide transitional housing for approximately 62 clients in four shelters located in Baltimore, Maryland.
Project PLASE operates four transitional housing facilities located at
- 201 E. North Avenue
- 139 E. North Avenue
- 2031 St. Paul Street
- 1814 Maryland Avenue
The
facilities house a total of 62 men and women at any given time. All
facilities are staffed 24 hours a day, seven days a week, year round.
All residents are provided three meals a day plus snacks, linens and
bus tokens, as needed as well as supportive services to assist with
their diverse needs.
The purpose of the transitional housing program is to house and service
homeless citizens who have special needs (namely AIDS/HIV, mental
health, psychiatric needs, chemical dependencies, developmental
disabilities, etc.), in a temporary housing situation with 24 hour
supported supervision, case management and counseling, with a focus on
helping the individual function at the highest level.
The following services are provided at Project PLASE Transitional Housing Facilities:

Three meals a day, seven days a week for every resident. Special
provisions will be made for people on special diets or nutritional
restrictions.

Linens (top
and bottom sheet, blanket, towels and washcloth) will be provided by
PLASE for each person. A supply of soap, shaving items, toothpaste,
feminine hygiene items, and shampoo will be kept for all in need. Good
hygiene will be reinforced and encouraged.

Each resident will be assigned a counselor at PLASE’s housing
facilities for assistance and support. At a minimum, they will meet
once a week.

Active
case management is given to assist the individual to meet his/her
needs, move beyond the crisis situation, and avail him/herself of all
community based services and resources as appropriate and necessary.

Program Description:
Through the federal Shelter Plus Care program, we provide permanent housing to over 100 single adults and families with disabilities throughout Baltimore city and Baltimore county. The program enables them to
live in private housing stock in the community while supporting
individuals to address all their needs in the areas of medical, mental
health, psychiatric and chemical dependency treatment.

Program Description:
Outreach counselors follow PLASE clients as they move into
community-based, long-term living situations. They provide support for
individuals to live as independently and fully as possible. This
includes access to the community and its resources and prevention of
recurring homelessness, institutionalization, or isolation. This
program serves the individual as an integrated move to becoming an
active member of the social community.

To house 200-250 Homeless citizens in our Transitional Housing Progra
By offering safe and stable housing paired with supportive services, PLASE provides the critical foundation for homeless men and women to gain personal stability and essential life skills. These new strengths enable many clients to transition to, remain in and reunite with their families in permanent housing. Cited in the Hopkins Research Summary of March 2008, "housing status is among the strongest predictors of entry into HIV care, primary care visits, continuous care, and care that meets clinical practice standards. [And] receipt of housing assistance has an independent, direct impact on improved medical care." Through individualized case management as well as through PLASE's many community partnerships and referrals, an overwhelming percentage connected with medical and other care.
