Population Served:

Program Description:

Case Management and Client Intervention – children and adults with disabilities are provided with community support enabling them to reach their own independence and quality of life goals.  Assistance may take the form of guided referrals, advocacy, or peer and professional support.
CLASS Case Management – case managers insure that children and adults who qualify for this Medicaid waiver program receive the supports needed for them to remain in the communities where they live rather than in institutions.  Service plans are based on each individual’s unique situation and personal choices, and may include attendant care, transportation assistance, durable medical equipment, home or vehicle modifications, and traditional and specialized therapies.
Homemaker Services – in-home helpers perform light housekeeping tasks to help ensure a safe and healthy living environment for people with disabilities and other special needs.
Personal Assistance Services – in-home help with bathing, grooming, dressing, eating, mobility, and other personal tasks enables children and adults with physical and intellectual disabilities to live in their own homes rather than in nursing homes.
Respite Care Services – primary caregivers for children and adults with disabilities get a temporary break from their responsibilities when trained professionals briefly take over the role.  Usually only a few hours each week or month, this short-term break can improve the caregiver’s emotional and physical wellbeing, reduce stress and enhance the quality of life for both the caregiver and the recipient.

Funding Needs

We continuously need help to fund our pediatric rehabilitation programs, our pediatric autism treatment programs, and our respite care programs for adults with disabilities. The need for each of these programs by individuals with low incomes is tremendous. Medicare and Medicaid do not fund treatment for autism treatment services nor do they fund respite care for individuals with Alzheimer's. Low income individuals with these disabilities have no way to pay for the services that they need.
