What we aim to solve
BCC is one of seven CCBHC's (Certified Community Behavioral Health Center) in Pennsylvania. This model of integrated physical and behavioral healthcare is person centered. It is a holistic approach, recognizing prevention and wellness are part of the entire treatment plan. Sustainability is contingent on further developing reachable, manageable outcomes that demonstrate a coordinated comprehensive array of services that are person centered and improve overall health and social conditions of an individual.

Certified Community Behavioral Health Clinic (CCBHC)
As a CCBHC/ICWC, Berks Counseling Center provides treatment services to include an integrated physical and behavioral health model of care inclusive of individual, group and family health and substance abuse counseling using evidence based approaches; psychiatric evaluation and medication monitoring including medicated assisted treatment (MAT) for opiate addiction and psychiatric rehabilitation services; screening, assessment and diagnosis including risk assessment; prevention and wellness programs; targeted case management and peer and recovery support services; specialized outreach to veterans, with a focus on outcome measures and data driven care.
Specialized Programs
Berks Counseling Center offers programs designed for Transition Age Youth, Pregnant and Parenting Woman, people with Serious Mental Illness, Substance Use Disorders and Opiate Use and family members needing services.
Housing
Permanent and Transitional housing for those who are homeless, have a disability, or are transitioning out of the criminal justice system.
Community Outreach
Berks Counseling Center offers outreach programs at Berks County Prison Community Re-Entry Center, Reading Area Community College, and Tulpehocken School District schools in addition to home, hospital and community visits to clients receiving services at BCC.

What is the organization aiming to accomplish?
BCC made available an integrated physical and behavioral, person centered, healthcare model of treatment for our community. Reading is a medically under-served area with a large Hispanic population (63.1%) of which 53.7% live in a non-English speaking household facing economic, cultural and linguistic barriers to healthcare. The goal is to improve health outcomes through onsite primary physical care, care coordination and nurse/care management, with an emphasis on prevention, recovery and wellness.
What are the organization's key strategies for making this happen?
At BCC, our staff is licensed and certified to deliver the array of services CCBHC's provide for. As a CCBHC we provide both outpatient and intensive outpatient care including health screening and monitoring and a person centered treatment plan. Our Medication Assisted Treatment (MAT) program helps those with substance abuse disorders. Assertive Community Treatment (ACT) program assist those ages 16-25 with a severe mental illness. Our housing and vocational services program offers transitional or permanent housing to homeless men, women and families in treatment and recovery and seeking employment. Peer Supports work with clients at the Decision Support Center offering guidance, mentorship and hope. Targeted case management, both in office and in the community assist clients with social service needs and help address the social determinants of health. Partnerships and collaborations are vital to success. Funding from Substance Abuse and Mental Health Services (SAMHSA), Office of Mental Health and Substance Abuse Services (OMHSAS) are critical to CCBHC' success. Service Access Management (SAM) provides crisis support off hours and in emergency situations, Threshold Rehabilitation Services provides psychiatric rehabilitation in two of their facilities. Partnerships with Community Care Behavioral Health (CCBH), Council on Chemical Abuse (COCA), Mental Health/Developmental Disabilities (MH/DD), AmeriHealth Caritas, National Council on Behavioral Health Care (NatCon), Berks County Jail Systems, Opioid Task Force, Berks County Veterans Affairs and local and federal government assist us in providing comprehensive, patient driven health and wellness.
What are the organization's capabilities for doing this?
Services are available to everyone in Berks county regardless of sex, race, religion, sexual orientation, gender expression or ability to pay.
Our focus offers a holistic wellness approach to a culturally and economically diverse population through physical - behavioral health integration. We rely on support from the healthcare industry, local and federal governments and the community.
What have they accomplished so far and what's next?
BCC has seen significant progress within the nine, CCBHC quality measures. Next steps include expansion of the CCBHC over the next two years to address issues identified but not funded through the CCBHC payment model. There are 4 defined goals of the expansion; 1) expand our capacity of the behavioral health workforce, services and infrastructure NOT included in the Clinic model but are a proven major benefit to our clients, especially wellness activities; 2) Increase access to screening and treatment for children and adolescents with Serious Mental Illness (SUI), Severely Emotionally Disturbed (SED) or on the autism spectrum by expanding our transportation abilities and training opportunities for clinicians working with these groups; 3) expand our capacity to support clients experiencing multiple social factors which mitigate their recovery including education, housing and employment and 4) pursue sustainability of physical health, wellness and prevention programs through enrollment in health insurance networks. All goals have measurable outcomes based on evidence-based practices.
