Triangle Community Center (TCC) works to create effective solutions to impact the health disparities experienced by the LGBTQ community in greater Fairfield County, CT. A priority need and issue TCC is working to address at this time is young adult homelessness in CT, and particularly the impact it has on LGBTQ youth and young adults. Whereas one-third of the 3000 homeless young adults in CT identify as being LGBTQ, studies show that 750 of these homeless young adults reside in Fairfield County. Triangle Community Center now has a 20 hour a week young adult drop-in center operated by trained staff from TCC's social service navigation program.

Social Service Navigation

Triangle Community Center’s (TCC) Case Management connects self-identified LGBTQ clients living in Fairfield County to programs and services to address their needs while fostering empowering relationships and seeing to safe and independent outcomes.

Vision Statement:

TCC’s Case Management is consciously aware of and diligently meeting the growing needs of the LGBTQ and Allied community in Fairfield County.
TCC’s Case Management continues to increase partnerships with agencies and service providers to ensure the highest quality of service delivery and connectivity for our LGBTQ clients.
TCC’s Case Management supports clients with ongoing counseling, life skills and provides mentoring as they move on to the next stages of their lives.
TCC’s Case Management Services recognize LGBTQ health disparities and prioritizes addressing these needs. Including but not limited to:

Triangle Community Center aims to end young adult homelessness in greater Fairfield County, CT as well as having a significant impact in addressing the many other health disparities that burden the LGBTQ community in our state.

Social Service Navigation, Community Health Assessment Surveys, Welcoming and Affirming Service Locations, Training for Local Social Service Providers, Collaboration with Agencies that Provide Important Resources for Our Community

We hope to continue our progress in 2018 now that we have a full-time staff of 6 and continue to pursue resources that will allow us to evaluate our current programming. Triangle Community Center's programs and services are lead by a licensed social worker with more than 10 years of professional experience in facilitating effective solutions to serve the needs of the LGBTQ community.

In the past five years Triangle Community Center has accomplished the following: hiring its first full time paid staff person, creating a social service navigation program, creating a training program for local social service providers, building a new welcoming and affirming drop-in center for our clients, creating Pride in the Park as the agencies premiere outreach event reaching thousands of LGBTQ people in our area each year.
