TOGETHER... making health and well-being a reality!
Programs and results
What we aim to solve
The problem we aim to address is the fragmentation of healthcare.
Our Vision and Mission statements take us out of the niche market of behavioral health, bypasses Primary Care as we know it, and plops us right at the forefront of Integrated Care. We want to set the standards, set the bar. While our story is complex, we wanted our mission to be simple. Everything we do is about our mission: Together...making health and well-being a reality!
Our programs
What are the organization's current programs, how do they measure success, and who do the programs serve?
Housing Program
We manage six HUD properties and two co-op properties. We use wrap-around services to assist with rentals.
Career Connections
Employment services that match qualified applicants with jobs at no cost to the applicants or the employers.
Addiction Services
We offer medication assisted therapy, various talk families, treatment for co-occurring disorders, and adolescent programs.
Deaf Services
Life skills instructors/coaches and
therapists are assigned to work with clients,
families, and social service agencies to assist
in treatment as needed and agreed upon by
the client.
HIV/AIDS Services
HIV Care Coordination is a specialized form of
case management. Its mission is to coordinate a
wide variety of health and social services to assist
those living with HIV/AIDS.
Indiana Works
A Work Incentives Planning and Assistance (WIPA) project, providing comprehensive benefits planning and work
incentive counseling to eligible Social Security beneficiaries.
Kids Talk
Kids Talk is a child-focused location where children can report abuse in a safe and comforting environment. Kids Talk provides protection and justice through a multi- disciplinary approach including law enforcement,
Department of Child Services, the prosecutor’s office,
mental health, healthcare, and trained advocates.

School-Based Services are those counseling services provided at a school facility. We treat children of all ages whose behavior and functioning has been impaired by a variety of symptoms associated with ADHD, anxiety, adjustment to trauma and loss, behavioral problems, depression, substance use, and thought disorders,
as well as other issues.
Wraparound Programs for Youth and Their Families
Wraparound is designed to combine formal supports such as therapy, life skills, medication management, etc., with natural supports in the home and community. Natural supports may include extended family, friends, pastors, neighbors and others involved with the family

Charting impact
Four powerful questions that require reflection about what really matters - results.
What is the organization aiming to accomplish?
Aspire Indiana is a fully integrated health system that addresses behavioral health, primary care, substance use disorders, and the social determinants of health, including housing and employment in Hamilton, Boone, Madison, and Marion counties in Indiana. We also provide HIV services, deaf services, veterans programs, and advocacy for abused and neglected children in Madison County.
Our mission statement is “Together...Making Health and Well-Being a Reality". That reality can only be achieved by taking healthcare out of its traditional silos of behavioral health, primary health, and substance abuse treatment and managing health from a whole person perspective. Our professionals work as a team to develop individualized and comprehensive treatment plans for the primary and behavioral healthcare needs of each individual that we serve. Many of our services are offered on a sliding-fee scale for those who don't have insurance.
What are the organization's key strategies for making this happen?
Our treatment plans for behavioral health and substance use disorders can include one-on-one counseling, group therapy, family therapy, skills training, and medication assisted treatment. Integrated primary healthcare providers can treat countless acute concerns as well as chronic conditions, like diabetes and high blood pressure, that require long-term management.
With over 500 members of staff and locations in Anderson, Carmel, Elwood, Indianapolis, Lebanon, and Noblesville, we are changing the way that healthcare is delivered not only to our target populations, but to anyone who wants high quality integrated healthcare at an affordable price.
What are the organization's capabilities for doing this?
Led by a forward-thinking administrative team, Aspire Indiana is ready to re-connect health and wellness services in order to become a large integrated care entity that includes all aspect of healthcare. We have set specific, measurable goals that are closely connected to our integrated care plan. Our goals include becoming a 100% cloud-based organization by the year 2019; helping people we serve with serious mental illness to live 10 years longer than they historically live helping them manage their illness; we hope to have zero suicides in Aspire services by 2021, and we expect to see 20,000 unduplicated clients by 2020.
What have they accomplished so far and what's next?
We are largely cloud-based at this time, and we are adding new centers and increasing the size at existing centers in order to meet patient and client needs.
