Mission

APC was founded in 2001 to provide a "medical home" to Arlington's medically underserved children, ages 0 through 18 years of age, whose family income is at or below 200% of the Federal Poverty Level. A "Medical Home" offers outpatient medical care that is comprehensive, accessible, continuous, family centered, coordinated, and compassionate. It is delivered or directed by pediatricians who manage or facilitate essentially all aspects of pediatric care. The physician develops a relationship of mutual responsibility and trust with the child and family. APC care is culturally competent, and interpreters are provided for the families who speak little or no English. All of our families face the daunting challenges of poverty. Most are recent immigrants from Central and South America, but families are also from Ethiopia, Mongolia, Turkey and other countries. They are unfamiliar with US practices and standards of health care. Too often patient care has been absent or delayed, and is complicated by psycho-social concerns caused by poverty, ignorance, illiteracy, cultural barriers, poor mental health and addictions. Intensive parent/patient education and case management is needed as a high percentage of our children are at risk for abuse and neglect. APC works closely with public health, school health, child protective services and other agencies to link children and families with needed services.

What are the organization's current programs, how do they measure success, and who do the programs serve?

Virginia Hospital Center Pediatrics

Each year, APC cares for more than 3,000 infants and children in Arlington County who live at or below 200 percent of the Federal Poverty Level. About 8.8% of APC patients are uninsured and the remaining 91.2% have Medicaid or FAMIS. Before coming to APC, many of our uninsured patients received insufficient medical care, leaving them more likely than other children to be in poor health. They are also more likely than their peers to suffer from asthma, mental illness, chronic illness, low birth weight, and developmental disabilities.

APC services include preventative and well-child care; acute care; mental health care; nutrition counseling; care coordination for Children with Special Health Care Needs (CSHCN); chronic disease management; case management; lab and radiology services; pharmacy assistance; language assistance; Medicaid and FAMIS enrollment assistance; care coordination with pediatric specialists, schools, and public health and human service agencies; and literacy development through our Reach Out and Read program.
