Our Mission To deliver quality, compassionate primary healthcare –every patient, every time –in response to the needs of our communities. Our Vision We are a model of affordable healthcare and a trusted partner, building healthy communities, one patient at a time. Our Economic impact: Advance Community Health provides about $4.5 million annually in discounted and free medical care, dental care and free drug prescriptions for patients who cannot afford to pay.
