What are the organization's current programs, how do they measure success, and who do the programs serve?
Primary Health Care
a. Providing primary treatment to 20 patients per day, 5 days per week, thru FIU’s Herbert Wertheim Medical School and Staff ARNP and Volunteer Physicians
b-Providing Tobacco Cessation counselling by certified Counselor in conjunction with University of Miami Miller School of Medicine
c- Providing childhood immunizations – such that all neighborhood children will be up to date on their immunization schedule by the end of the year
D-Providing yearly Eye examination to prevent Retina Damage in conjunction with Bascom Palmer Eye Institute
The service are evaluated and monitored through several mechanisms. Volunteers assist in the gathering of statistics on usage both daily and compiled. Specific services are tabulated, such as: Children immunized at the center as well Tobacco Cessation cases. These data are provided to Blue Foundation, Department of Health as well University of Miami’s Miller School of Medicine
Patients screened and/or treated for diabetes and high blood pressure. Referrals to more advanced care is provided.

What is the organization aiming to accomplish?
To provide free healthcare services to the underserved population of South Florida regardless of their Ethnicity, National Origin, Sexual Orientation or Religious and Political Affiliation.
UHI CommunityCare Clinic plays a vital and unique role in meeting the healthcare needs of the underserved communities it serves, with a particular focus on addressing primary healthcare, mental illness, and food insecurity. Several distinguishing features set UHI Clinic apart from others providing similar services in the community:
Integrated Primary Healthcare Services: UHI Clinic offers comprehensive primary healthcare services, serving as a one-stop destination for residents' medical needs. By providing routine check-ups, preventive care, vaccinations, and chronic disease management, the clinic addresses the fundamental healthcare needs of the community. The integration of various healthcare services under one roof promotes continuity of care and reduces barriers to accessing essential medical attention.

The sustainability of UHI Clinic's activities depends on a well-thought-out plan that encompasses both staffing and financial aspects. To ensure the clinic's long-term success, the following elements are part its sustainability plan:
Diversified Funding Sources:
Beyond relying solely on grant funding, UHI Clinic actively seek diversified revenue streams. This includes in-kind partnerships with HW School of Medicine and Nursing at Florida International University, and other social organizations.
Grant Writing and Fundraising:
Dedicated staff with grant writing experience and fundraisers helps secure grants and donations to cover operational costs.
Hosting regular fundraising events and support from the Board of UHI Clinic generate financial support.
Volunteer and Internship Programs:
Utilizing volunteers reduces staffing costs while maintaining the quality of care. These individuals are from local schools to earn community service credits.
Providing opportunities for medical and nursing students from Florida International University to gain practical experience also fosters future healthcare professionals who stay involved with the clinic.
What are the organization's capabilities for doing this?
A board of directors with expertise in healthcare management, finance, and community development provides guidance and connections for the clinic. Board members also assist with fundraising efforts as well as providing 40-50% of the clinic’s operating expenses.
Cost-Effective Operations:
Continuously evaluating and optimizing clinic operations for efficiency to reduce costs.
Utilizing electronic health records (EHRs) and telemedicine services streamlines administrative tasks and expands the reach of healthcare services. Utilizing an outsource company to provide front end services has helped contain administrative expenses.
Health Education and Community Engagement:
Investing in health education programs and community engagement initiatives helps prevent illnesses and reduce the overall burden on the clinic. Partnering with Miami Dade Fire Rescue to provide regular educational workshop on First Aid, Signs and early detection of Stroke as well helping choking victims
Partnering with National Association of Mental Illness of Florida (NAMI-Fla) to educate community at large on early signs and prevention of Suicide specially among youth.

Established in 2008 as 4 hours per week free clinic is now full time free clinic providing multiple services

Primary Care, Podiatry, Optometry, Psychiatry, Pain Management, Nephrology, Pediatrics, ENT, Childhood Vaccination, Flu Shot, Laboratory Services, Mental Health Counselling, Food Insecurity support, Free Eyeglasses.
Accomplishments
- With Herbert Wertheim School of Medicine and Nursing at Florida International University to train their Medical and Nursing Students at the clinic
- with Miller School of Medicine at University of Miami to help with Smoking and Tobacco Cessation
- with MAVEN Projects (htps://www.mavenproject.org/) to provide
specialty medical care
-only Free Clinic in South Florida to be selected by Quest Diagnostics to provide free Laboratory tests.
PARTNERSHIPS
-Partnership with Islamic School of Miami (ISOM) and Islamic Foundation of South Florida (IFSF) food pantries to help with food insecurities while providing holistic support to underserved community
-Partnership with Jewish Community Services (JCS) to provide 24/7 Mental Health Services
-Partnership with Miami Dade Fire Rescue to provide workshop training on First Aid, AED and early signs of detecting Stroke
-Partnership with National Association of Mental Illness- Miami Dade (NAMI) to provide workshop on Prevention of Suicide.
-Jackson Health System- reciprocal patient referrals
-Bascom Palmer Eye Institute-Reciprocal Patients referral and yearly exams for Degenerative eye dieses
-Florida Association of Free and Charitable Clinics
-City of Miami Gardens Healthy Living Initiatives.
