Dental Care
CHC offers preventive dental services provided by a
licensed dentist or dental hygienist, including:
oral hygiene instruction; oral prophylaxis, as necessary; and Topical application of fluorides, and the prescription of fluorides for systemic use when not available in the community water supply.
Medical Care
Community Health Centers,
Inc. provides, directly or through contracts or cooperative arrangements, basic
health services including: primary care; diagnostic laboratory and radiological
services; preventive services including prenatal and perinatal services; cancer
and other disease screening; well child services; immunizations against
vaccine-preventable diseases; screening for elevated blood lead levels,
communicable diseases and cholesterol; eye, ear and dental screening for
children; family planning services and preventive dental services; emergency
medical and dental services; and pharmaceutical services.
Behavioral Health
CHC provides treatment for depression and related illness.
Healthy Outcomes Prenatal Program
The Healthy Outcomes Prenatal
Program is an innovative prenatal program which takes a positive, proactive
role in ensuring a healthy pregnancy. Based
on the premise that community-driven strategies are needed to address factors
contributing to infant mortality, low birth weight, and other adverse perinatal
outcomes in high-risk populations, our program focuses on improving maternal
and child health outcomes by increasing access to and use of health services
for women.
The program is designed to provide low-income pregnant
women with a comprehensive program of prenatal care from conception through 60
days postpartum.The comprehensive
package of services for pregnant women includes medical, nutrition, and social
services that promote healthy birth outcomes.Services will be tailored to the needs of high risk pregnant women in
geographically, racially, ethnically, and linguistically diverse communities
with exceptionally high rates of infant mortality and/or low birth-weight rates.
Their provider will write an
appropriate care plan complete with goals and interventions. The provider will perform periodic assessments
to monitor the member’s physical status and emotional needs.
