Mission

Access TLC Foundation provides financial support to low income and indigent patients and their families, to improve the quality of life at the end-of-life. In addition, the Foundation provides education to the community and continuing education to medical staff relating to hospice care. We provide support funding for caregivers, periods of respite, assistance with medication and supply costs, transportation and other services and supplies deemed necessary for end of life critical care. We also support the families with bereavement counseling, help with memorials and cremation costs,

What we aim to solve

Low income, under insured & medicare Patients with terminal illnesses who are in need of end of life hospice care face medical expenses that are not covered by insurance or medicare are in dire need of financial assistance to provide them with the care, support and medications they need to manage symptoms related to a terminal illness. Many of these patients and their families struggle to pay for these services, and maintain utility bills as well. Most Hospice care agencies provide whatever care that is covered by insurance or medicare only. Access TLC also assists with the costs of other financial needs such as electric or gas bills, food etc on a case by case basis. We not only support the patient but the families as well by offering financial assistance in memorial and cremations costs, as well as bereavement counseling.

Low Income & indigent patient financial support

Access TLC Foundation provides financial assistance for medical hospice care (skilled days in nursing facilities, caregiver care for the "home-bound", palliative and respite care, medications & supplies not covered by insurance, assistance with memorial and cremation costs and grief counseling for the families.
The program also provides Community education on hospice and end of life care as well as continuing education for medical staff.

Population(s) Served

Financial support is available through the Foundation to individuals receiving end-of-life care. The patient or their primary caregiver can apply for funding by completing and submitting a Application of Needs Form. Following review, assistance is given to those who are identified as FPL and meet the critical care and support needs.

Access TLC Hospice Foundation is unique in the Hospice community by having a commitment to offer quality care and programs that not only exceeds expectations but truly makes a difference in the patients physical,mental and spiritual health. Our goal is to increase the number of patients we will be able to provide financial support for their end of life hospice care.

we are actively searching for Grant givers who have a mission to assist those who are low income, indignant and elderly with terminally ill prognosis receiving end of life hospice and respite care. Access TLC Hospice Foundation strives to assure that we can assist as many people as possible to provide quality of life..at the end of life. We also offer family members of the terminally ill bereavement counseling and burial assistance, General hospice education in our community is an on going part of our services as well as continuing education for medical staff.

Since it's inception in 1996 Access TLC Foundation’s has provided financial support to improve the quality of life at the end-of-life, for patients and their families demonstrating financial need or indigent in Ventura and Los Angeles County.
The Foundation is governed by a Board of Directors comprised of volunteer community members who are selected for their commitment to the local community, hospice care services and end-of-life care for all. Additionally, the Foundation receives the support of a group of dedicated volunteers who help with fundraising activities and administrative duties. Through the support of many individuals who are avid donors to our mission as well as several annual fundraising events our Foundation has been able to provide these services for over 20 years!
Boosting the Foundation’s capacity is its sister agency Access TLC. The for-profit health care agency strides to provide excellence and growth in ways that help people have a better quality of life.

During the last year our patient financial assistance have increased 3 fold as a result of our continued outreach and community education programs. Due to the increase in the aging Boomer population and increasing number of elderly affected by life ending diseases such as Alzheimer, Dementia and Cancer, we are setting a new goal extend our end of life support services for the residents of Los Angeles County. Our present goal is to provide income eligible seniors with resources and care otherwise not available to them due to not being covered by insurance and/ or financial constraints. This program will prioritize "end-of-life" seniors with objectives of keeping them in their home and among their loved ones as long as possible; as well as maximizing quality of life.

We are seeking grants to provide the extra funding needed to accomplish this goal. Prospective beneficiaries of our program will undergo an assessment of their financial needs not covered by insurance. These needs will be prioritized and budgeted based on an initial $2000 allocation of funding. The allocation will be used to provide care giving and other services not covered by insurance,
