Health & Wellness

The Road to WellnessWe promote positive aging. Our vision is to help cultivate caring, supportive communities where we can all age gracefully and with dignity.

We are a social profit organization that is funded primarily through grants from the Federal Government, the Department of Health, Albuquerque Area Agency on Aging, the Non-Metro Agency on Aging, and United Way.

We receive funding through the government because we are non-discriminatory. Our services are for people from all religious and ethnic backgrounds, young and old, urban and rural. We encourage everyone to come to our events!
ActivitiesWe try to cater to your community as much as possible. Every community is unique and we prefer to work with you to develop a program that uniquely enhances your community. Here are some of the programs we offer at many of our sites:

Population(s) Served

The Case Management program supports individuals and their families facing difficult times in their lives. Our experienced and compassionate professional staff conducts interviews or, if appropriate, comprehensive in-home assessments to determine client needs. Our professional staff then links clients to appropriate services and provides follow-up and further coordination as needed.

The Case Management program also refers to social services accessed through the City of Albuquerque/Bernalillo County Department of Senior Affairs.

JFS is here to assist you if you are experiencing difficulty with:• Aging-related issues• Housing• Navigating social systems (Medicare, Medicaid, Income Support Division)• Obtaining increased levels of care

SERVICES

JFS case managers offer a wide scope of services, including:• Advocacy• Information and assistance• Assessments• Coordination of service with providers• Family caregiver support• Education• Medical case management (medication set- up && nbsp management, medical advocacy)

Population(s) Served

The Senior Transportation Project (STP) provides van transportation assistance to seniors for medically related appointments, grocery shopping, mall shopping and social activities. STP helps minimize the isolation experienced by many seniors and increases their access to community services and activities.

STP provides door-to-door van transportation, by appointment, for adults age 60 and over. A wheelchair accessible van is available.

Hours of operation: Monday through Friday from 8:30 a.m. to 4:30 to the east by the foothills, to the south by Gibson Ave., to the southwest at Coal Ave, and to the west by Rio Grande River. We will take clients to the VA hospital.

Jewish Family Service’s (JFS) In Home Services program provides housekeeping and companionship services, and handyman services to seniors who are experiencing difficulty at home or just need a little extra help. A Fee-for-Service program is available to anyone at an hourly rate with a two hour minimum per visit. We also provide service on a sliding-fee scale available through program grants for clients of the JFS Case Management program.

Population(s) Served

Modeled on successful programs in other cities, the Community Chaplaincy program focuses on providing direct pastoral care visits to people who are ill, in crisis, or nearing the end of their lives - in hospitals, nursing homes, or at home in hospice care. The program provides spiritual support to families and caregivers.

Services also include spiritual counseling and bereavement support. The education component of the program teaches about Jewish traditions such as ethical wills, holiday observances and funeral practices, as well as talking with healthcare and funeral home personnel about the needs of Jewish clients. Interfaith activities and coordination with chaplains of other faith communities are integral components of the program.

As part of our role to strengthen the Jewish community, the Community Chaplain participates in activities with congregations and other Jewish organizations in the community.

In addition, the Community Chaplain oversees the Jewish Sacred Society - Chevre Kaddisha
